Benefits enrollment packets are on the way
Enrollment materials for the 2005 NUFlex benefits plan will be sent to UNL employees Nov. 12. Employees may make changes to their benefits packages by returning the forms or online through the Employee Self-Service Web site.
The packets will include all information about the NUFlex plans offered this year, but here are some details:
Medical Insurance:
• Prices for Blue Cross Blue Shield coverage will increase $2 to $12 per month depending on the specific medical plan and coverage category chosen. The average increase for all options combined is a net out-of-pocket cost of about $6 per month.
Caremark Prescription Drug Program:
• The copay for prescription drugs will change to the following for each 30-day prescription: Brand (on formulary/primary drug list), $28, from $27 last year; Brand (not on formulary/primary drug list), $45, from $43 last year. The $10 copay for generic prescription drugs remains the same as last year.
Dental:
• Price tags will not change in 2005.
• The dental plan chosen during the 2004 NUFlex enrollment will stay in effect through Dec. 31, 2005.
Vision Care:
• EyeMed vision care prices will increase $1 to $2 per month.
• The vision-care plan chosen during the 2004 NUFlex enrollment will stay in effect through Dec. 31, 2005.
NUFlex enrollment forms only need to be returned if the employee wants to change current NUFlex choices or to enroll in the health or child-care reimbursement accounts.
Change forms must be returned to the UNL Benefits Office by Dec. 3.
After the enrollment deadline has passed, all employees will receive a confirmation statement of their NUFlex benefits choices. Employees should review these statements and notify the UNL Benefits Office of any errors or problems.

Meetings to answer questions about NUFlex enrollment will be:
• Nov. 17, 10-11 a.m., Nebraska Union
• Nov. 19, 10-11 a.m., Nebraska East Union
• Nov. 30, 2-3 p.m., Nebraska Union
• Dec. 2, 2-3 p.m., Nebraska East Union
Employee Self-Service
This year, employees can use the Employee Self-Service Web site to submit NUFlex benefit changes online.
Instructions for completing enrollment online will be included in the benefits packets. Users must have a user ID and password. Employees who need new passwords can call the UNL Help Desk at 472-3970 to get passwords reset. ESS is not accessible using Mac computers or Netscape Navigator Web browsers.
